The 5-level QA maturity model
This model describes testing maturity across five dimensions: process, automation, metrics, collaboration, and continuous improvement. Each level builds on the previous one. Skipping levels creates fragile practices that collapse under pressure.
Level 1: Ad-hoc (Reactive)
Testing happens, but without structure or consistency.
Process indicators:
- No documented test strategy or test plans
- Testing scope decided individually by each tester based on experience

- No defined entry or exit criteria for testing phases

- Developers and testers work in silos
- QA learns about requirements from completed code, not from planning sessions
Self-assessment: If more than half of these describe your team, you are at Level 1.
Level 2: Defined (Structured)
Testing follows documented processes with consistent execution. Key indicators: written test strategy, test cases in a management tool (TestRail, Zephyr), bug tracking with severity classification (Jira, Linear), defined entry/exit criteria, consistent release checklist, managed test environments, unit test suite at 40-60% coverage with CI execution, build fails on test failures, defect count tracked per release, and test execution progress visible to stakeholders.
Collaboration indicators:
- QA participates in sprint planning and requirements review
- Developers and testers communicate through shared tools, not ad-hoc channels
- Definition of Done includes testing criteria
Self-assessment: If most of these are in place, you are at Level 2. The priority is expanding automation and establishing data-driven practices.
Level 3: Integrated (Automated)
Automation is the primary regression strategy and testing is integrated into the development workflow.
Process indicators:
- Risk-based test planning: test effort allocated by feature risk, not uniformly
- Regression suite maintained and executed automatically every release
- Exploratory testing sessions scheduled and documented (session-based test management)
- Non-functional testing (performance, security, accessibility) included in the process
- Test data management strategy in place
Automation indicators:
- Automated regression suite covering 60-80% of critical paths
- CI/CD pipeline includes unit, integration, and E2E test stages
- Quality gates block deployment on test failures
- Test automation maintained as code: version controlled, code reviewed, refactored
- Flaky test management process in place (quarantine, fix, restore)
Metrics indicators:
- Test automation coverage tracked and reported
- Defect escape rate measured (defects found in production vs pre-production)
- Test execution time tracked and optimized
- Release cycle time measured (from code complete to production)
Collaboration indicators:
- Developers write unit and integration tests as part of Definition of Done
- QA engineers focus on test design, automation, and exploratory testing
- Shift-left practices: QA reviews requirements and provides testability feedback
- Cross-functional quality discussions in retrospectives
Self-assessment: If most of these are in place, you are at Level 3. The priority is using data to drive decisions and building predictive quality capabilities.
Level 4: Measured (Data-Driven)
Quality decisions are based on data analysis and testing effort is optimized through metrics. Key indicators: risk models quantify testing priorities based on change frequency and defect history, 80-90%+ automated regression coverage, performance and security testing (SAST/DAST) automated in CI/CD, test infrastructure provisioned as code, defect prediction models in use, quality dashboards visible to the entire organization, cost of quality tracked (prevention vs detection vs failure), QA engineers act as quality coaches, and production monitoring insights feed back into test design.
Self-assessment: If most of these are in place, you are at Level 4.
Level 5: Optimizing (Continuous Improvement)
Quality processes continuously evolve based on feedback loops. Key indicators: testing strategy reviewed quarterly, AI-assisted test generation and self-healing tests, intelligent test selection (only tests affected by code changes), chaos engineering as regular practice, predictive quality analytics, business impact correlation (quality metrics linked to revenue and retention), quality culture embedded beyond engineering, and customer feedback directly influencing testing priorities.
How to conduct the audit
Week 1 — Self-assessment. Distribute this checklist to QA team members, development leads, product owners, and DevOps engineers. Ask each person to rate every indicator as fully in place, partially in place, or not in place. Collect responses anonymously.
Week 2 — Gap analysis and level determination. Compare responses across roles. Disagreements reveal perception gaps worth addressing. Map indicators into three categories: strength, gap, and inconsistency. Your maturity level is the highest level where 80%+ of indicators are fully in place.
Week 3-4 — Target setting and roadmap. Set a target one level above your current state for 12 months out. Prioritize gaps from your current level first, then the next level. For each gap, define an action, owner, deadline, and success metric. Group into quarterly milestones.
